To this day I still feel guilty about the ME2 suicide mission and my decision that Dr Chakwas and the crew members with her should find their own way back to the Normandy. Of course, they were all slaughtered and ended up on the Normandy memorial wall. I was so focused on the mission that I made a grievous command decision when it came to their fate! Much to my chagrin, I read later that I could have assigned them an armed escort to get them back to the ship safely.
